Admission and Enrollment

The PAC² Center provides child care for children six weeks through 13 years of age. There is a one-time enrollment fee of $35 required upon enrollment to cover administrative fees. If a child does not attend for a period of three months or more they will be required to re-enroll.

If you wish to have your child or newborn put on a waiting list for guaranteed enrollment at a later date, we require a deposit of the first week’s contracted rate plus $35 enrollment fee.

In addition, the following forms are required prior to placement.
● Registration Form
● Immunization / Child Health Record
● Transportation Agreement
● Medication Administration / Competency
● Photograph/Video Consent Form
● Child Release Form
● Food Program Qualification Form
● Handbook Receipt/Policy Agreement Form
● Walk Permission Form

Parents need to keep the Center informed of any change in address, telephone, or other pertinent information included on the above listed forms.
